Seeking SOCIAL WORKERS/HEALTH CARE INTEGRATORS for our BRIDGES TO HEALTH (B2H) - Lower Hudson Valley Program


This Position requires travel throughout Lower Hudson Counties


OFFICE IS IN IRVINGTON, NY



Requirements:


MA/MS in Human Services + 1 yr service coordination experience or BS/BA in Human Services + 4 yrs experience required.
Must be able to pass CANS-NY training (Children and Adolescent Needs and Strengths)
Knowledge of Connections or Excellent Computer Skills
Valid Driver's License & Car


Job Summary:

The HCI is responsible for Health Care Integration, the B2H Waiver service through which the Individualized Health Plan (IHP) is created and managed. Their role is to assess the need for services, develop and update the IHP, link the child and family/caregiver to the identified services, consult with the child and caregiver on the appropriate provision of services, stabilize the environment for the child and caregiver, and advocate for the child's needs. From information supplied by the Local Department of Social Studies (LDSS) and obtained from the child, medical consenter, and caregiver, the HCI identifies the health care service needs of the child and arranges for the necessary medical, social, rehabilitative, vocational, and educational services that the child needs. The HCI must link with the caregivers and professionals associated with the child's IHP throughout the child's enrollment in the B2H Waiver program, while the child is a client of the HCIA. The HCI is responsible for monitoring the Waiver Service Providers' (WSPs) work in relation to the requirements of the stated goals in the IHP. The HCI is expected to intercede on behalf of the child or between the child and the WSP's. The HCI must also monitor and observe the child, family/caregiver and WSP's to confirm that needed health care services and supports are delivered.
