SEEKING CASE PLANNERS FOR OUR NYC FAMILY FOSTER CARE PROGRAM
Office is located in Bronx, NY
REQUIREMENTS:
Minimum BSW or BA/BS in human services + 1 year foster care experience
Excellent communication skills both oral and written
Must be able to pass CANS-NY training (Children and Adolescent Needs and Strengths)
Excellent organizational skills
Knowledge of ACS systems and CONNECTIONS
Candidate must be able to demonstrate the ability to reach out and work with community resources and foster parents.
JOB SUMMARY:
The Case Planner is responsible for the delivery of services to children in care and their families. This includes the coordination of community and school activities and services. The Case Planner has primary coordination responsibility for all community, school and family activities. He provides direct casework counseling for the child and his family. He represents the agency in all court procedures including the coordination of adoption placements as required by the case.
DUTIES & RESPONSIBILITIES:
Responsible for initiating contact with the foster parent and new intake within 48 hours of placement.
Responsible for assisting with the placement of the child in school.
Responsible for working with the child's family. This includes outreach efforts initiated within the first seven days of placement, and includes all diligent efforts required by ACS/OCFS regulations and other efforts determined to be appropriate by the supervisor.
Responsible for planning and coordinating all community contracted programming.
Responsible for planning and coordinating all therapy, counseling and other necessary therapeutic services for the child and for the biological family.
Responsible for meeting with the foster parents on at least a monthly basis in the foster home to discuss the child's progress in the foster home, and to assist with training of the foster parent in coping with acting-out behavior
Responsible for coordination of the transfer/discharge of each child.
Responsible for a variety of record keeping functions as dictated by State and City regulations such as: FASPS, Discharge Summaries, Psycho-Socials and accurate progress notes. All information must be kept current in the CONNECTIONS system within 15 days current.
Responsible for completion of Permanency Hearing Reports through the LTS system.
Responsible for maintaining a professional relationship with each child on his caseload as well as with the foster parents and with the birth parents.
Responsible for pursuing continued in service education including weekly supervision.
Responsible for developing and executing permanency planning with families that is consistent with supervisory meetings and with ACS and OCFS.
Responsible for any other related duties as required.
