Medicaid Service Coordinator
{Nassau, Suffolk & Queens}
Responsibilities:
•Provide linkage, referral, service planning and coordination services to individuals diagnosed with developmental disabilities.
Requirements:
•Have a min. of (1) year of exp working with DD population or (1) year exp. in case mgmt
•BA/BS in Human Services or related field a MUST
•Strong skills - MS Word; written and verbal
•Drivers License preferred
