Job Description:

Job Summary:

Provide direct care and consultation of symptom and other data associated with illness for enrollees, whether derived from monitoring technologies or from live (i.e. enrollees, family caregivers, providers or clients) sources, with the goal of imparting psychosocial interventions, education and coaching and to promote appropriate use of resources to affect the best clinical outcomes of enrollees, including self-care and the transition to stabilization or independent status. This individual will hire and manage other social/community workers, help plan and develop operation and act as leader going forward.

Responsibilities:

Conducts needs assessment / reassessments of new/enrolled patients in their home or other location (hospital, SNF, etc). Follows up on site visits as indicated by team member assessment – monthly, quarterly and as needed.
Provides teaching/coaching relevant to managing the enrollee’s illnesses and physical and psychosocial well-being utilizing the case management process to set priorities, plan, organize, and implement interventions that are goal directed toward self-care/long term care outcomes.
Advocates for the member/patient and family among various sites to coordinate resource utilization. Assesses that referrals have mitigated barriers and/or triggers as expected.
Collaborates with the patient, family, physician, agency and all members of the healthcare team, internal and external to this organization, striving for continuity and efficiency as the patient is managed along the continuum of care.
Coordinates the delivery of high quality, cost-effective care based on a customized model of care supported by clinical practice guidelines and policies and procedures in accordance with the MSW scope of practice.
Provides oversight of care plan to ensure execution and successful goal attainment.
Documents accurately and comprehensively based on the standards of practice and current organization policies.
Utilizes a team approach to provide services and collaborates and communicates with team members when needed.
Identifies when a program participant’s needs are outside the scope of the social worker’s practice and expertise, plus arranges for additional services to meet those needs.
Understands fiscal accountability and its impact on the utilization of resources, proceeding to stabilization and self-care outcomes.
Evaluates care by problem solving, analyzing trends and variances and participating as a team member in the quality improvement program to enhance client outcomes.
Accountable for program development and measurement, such as behavioral health and community referral resources for comprehensive care coordination.

Qualifications:

Education/Experience:

Current license as a Medical Social Worker in the appropriate jurisdictions (New York). This candidate will be required to apply for licensure in other states as needed.
Master's degree from a school accredited by the Council on Social Work Education.
A minimum of five years social work experience in a health care setting with people with chronic illness such as diabetes, heart disease, respiratory illnesses, preferably home health care, case or disease management.
Knowledge, Skills and Abilities:

Demonstrated knowledge of behavior change and motivational interviewing theory.
Self starter, with sound judgment, entrepreneurial spirit with strong work ethic.
Ability to develop a collaborative, therapeutic alliance with individuals.
Requires excellent organizational skills.
Requires excellent oral and written communication and presentation skills.
Requires excellent PC skills and the ability to utilize Microsoft Office applications (Excel, Access, Word, etc.), must be comfortable with technology.
Requires excellent problem solving and conflict resolution skills.
Requires strong engagement, interviewing and teaching skills.
High caliber on both the operational and strategic level and may be more senior level (VP) position
Must understand social services available on a scalable basis
Prefer multilingual ability.
Requires flexible work hours, time scheduling, travel to areas where patients are located, or on call work specific to the comprehensive care management program.
