-Valid Home Health Aide or Personal Care Aide Certificate
-Valid Physical (within six months)
-Valid Drug Screening with lab reports (within six months)
-Immunizations (Rubella/Rubeola)
-PPD (or chest x-ray)
-Photo ID
-Social Security Card
-2 Reference Letters
